I received this as a Christmas gift from my sister, CCRider2000 in 2013, along with a whistle/compass that i take on my inflatable kayak.  I couldn't find anything to affix the tag to so I'll just send it on it's way... it's mission will be to hopefully not get lost as every other tag I own has!

Apparently Mr. Martens misplaced it! He had a bad car accident awhile ago (2015?) and asked me to be patient... I hope he recovers and does something with this trackable.
