To complete this puzzle, you will have to answer the questions below, put them in order in the blank spots, decode it using the decryption key provided, and put the decoded letters and numbers in order in the blank spots.

You can validate your puzzle solution with certitude.

The answers are provided for questions numbers 13 and 14.

To find the decryption key, go to the photo gallery and find the photo entitled "Decryption Key". Each letter on the top corresponds with the number directly under it.

Directly below are the blank spots that need to be filled in. Once you have answered the questions, you can fill in the blanks from left to right according to what number the question is. For example, question number one would be the first blank space, number two would be the next, and so on. I have included two copies so that after you have answered the questions, you can fill in the first copy with the letters, and after you have decoded the letters to numbers, you can write them in into the second copy.
