The Tenth Doctor was cheerful, charismatic, hyperactive and enthusiastic, often at inappropriate times, but would become serious and demanding when the situation called for it. He tended to keep a massive smile on his face and simply enjoyed his journeys through time and space. The Doctor's humane side made him a more loving and fun character to be around, being very much a good friend to all his companions and welcomed anyone with open arms. He became almost like a family man, especially with the Tyler family.

While he seemed to have resolved much of the survivor's guilt felt previously, the Tenth Doctor had begun to feel his age. Beneath his juvenile exterior was an old man who had seen many terrible things and felt a deep loneliness. He felt profound regret for the deaths he'd seen, with the Moment referring to him as "the man who regrets". His keen sense of loss led him to empathise with those who had also suffered, and feel driven to prevent death and destruction wherever possible, having a hard time accepting failure. Donna Noble noted the reason the Doctor required a companion was to keep him from succumbing to his darker side.

The Tenth Doctor's main personality flaw was his ego, with him always trying to take control of situations he found himself in, and feeling it his place to punish those who committed horrific acts, such as when he deposed Prime Minister Harriet Jones, thus negating the "Golden Age" she was destined to achieve, for committing what he saw as an act of murder. Occasionally, he stated himself to be at a higher authority than he actually was, and believed himself to often be the smartest being in the room. He regularly used his name as a threat.

The Doctor's attire was often accompanied by a light brown Duster-type overcoat, which the Doctor claimed had been given to him by Janis Joplin. The Doctor had two main suits, a dark brown suit with blue pinstripes, and a blue suit with red pinstripes. Under his suits he usually wore a dress shirt and a loose tie under his unbuttoned collar, or wore an open-necked shirt with a T-shirt and white or grey top underneath.
