Ricordata come pieve già dal 1073 con il titolo di San Jerusalem, fu chiesa madre dell'importante piviere di Diacceto. Possesso dei vescovi di Fiesole già dal 1103, ebbe anche il patronato della famiglia Cattani da Diacceto e successivamente quello dei Moresi. Nel XII secolo la chiesa poteva contare su ben nove suffraganee. Successivamente, però, la pieve perse rilevanza e cadde in rovina, fino a quando, nel 1413, dovette cedere il titolo di pieve alla chiesa di San Clemente, nel quale fu spostato anche il fonte battesimale. Successivamente, dopo un restauro realizzato a metà del Cinquecento, il fonte battesimale fu tuttavia riportato a San Lorenzo a Diacceto.
Il vecchio edificio fu completamente demolito e ricostruito fra il 1858 e il 1872 con la trasformazione dell'originario impianto a due navate in un un'unica ampia aula. In quell'occasione la volta fu affrescata da Ferdinando Folchi e Antonio Noferi. La facciata a due spioventi fu rifatta in seguito ad un intervento di restauro del 1950 durante il quale furono aperte le monofore della parete destra e rimossi gli altari ottocenteschi. All'interno si conserva una terracotta invetriata che raffigura Maria in trono col Bambino e i santi Giovanni Gualberto e Antonio Abate contornata da angeli, attribuita alla bottega di Giovanni della Robbia (XVI secolo). Da segnalare anche, sulla parete destra, all'interno di un tabernacolo, il dipinto del primo Seicento che rappresenta la Madonna del Buon Consiglio.

Remembered as a parish church as early as 1073 with the title of San Jerusalem, it was the mother church of the important parish of Diacceto. Possession of the bishops of Fiesole as early as 1103, it also had the patronage of the Cattani da Diacceto family and later that of the Moresi. In the 12th century the church could count on nine suffragans. Subsequently, however, the parish lost its importance and fell into ruin, until, in 1413, it had to give the title of parish to the church of San Clemente, in which the baptismal font was also moved. Subsequently, after a restoration carried out in the mid-sixteenth century, the baptismal font was however brought back to San Lorenzo a Diacceto.
The old building was completely demolished and rebuilt between 1858 and 1872 with the transformation of the original two-nave layout into a single large hall. On that occasion the vault was frescoed by Ferdinando Folchi and Antonio Noferi. The gabled façade was rebuilt following a restoration in 1950 during which the single lancet windows on the right wall were opened and the nineteenth-century altars removed. Inside there is a glazed terracotta depicting Mary enthroned with the Child and the saints Giovanni Gualberto and Antonio Abate surrounded by angels, attributed to the workshop of Giovanni della Robbia (16th century). Also noteworthy, on the right wall, inside a tabernacle, is the early seventeenth century painting representing the Madonna del Buon Consiglio.
