La denominazione di questa chiesa di campagna deriva probabilmente dall'esistenza di un olmo nelle vicinanze.
Al suo interno l’edificio protegge un’edicola votiva di stile gotico affrescata da Vincenzo Pagani e raffigurante la Crocifissione.
La devozione dei fedeli indusse la comunità a racchiudere questa edicola in una piccola chiesa, poi ampliata nel 1521, nella quale fu eretta la Confraternita del Crocifisso.
In stile romanico e a pianta rettangolare, la chiesa presenta due altari, uno costituito nella parte anteriore dalla primitiva edicola e dietro uno successivo.

The name of this country church probably derives from the existence of an elm in the proximity.
Inside, the building protects a Gothic style votive shrine frescoed by Vincenzo Pagani and depicting the Crucifixion.
The devotion of the believers led the community to enclose this shrine in a small church, later enlarged in 1521, in which the Confraternity of the Crucifix was erected.
In Romanesque style and with a rectangular plan, the church has two altars.
