Relatively easy to find. You are looking for an ammo box with the usual trinkets and a log book.
This cache is placed at the One Man Cemetary site just outside of New Lisbon. The name on the memorial is Vincent Synkiewicz 1861 - 1938. The New Lisbon Lions Club maintains this site. I am hoping that by placing this cache someone who has more information about this burial site will either contact me or list the information in their logs when they visit.
