You can get to the cache location by boat or by land. If you go by water (which is perhaps more appropriate given the theme of this cache, but not the most practical), you will have to get out of your boat to retrieve the cache. You will be within a few feet of it by that time.
If you change the hemispheres on the coordinates for Pantelleria Island in Italy you will arrive near your destination. For example, change North to South, East to West.
