Jak podają źródła historyczne, pierwotnie w herbie miasta były pierwotnie trzy krzewy winorośli ze zwisającymi winnymi gronami. Jest to nawiązanie do winnic, które na południe od zamku, na stokach doliny rzeki Wogry, założył prawdopodobnie Hasso von Wedel, zwany Rudym, na wzór słynnych winnic katedralnych w jego rodzinnym mieście Soldin.
Warto podkreślić, że około 2 km na południe od zamku (po prawej stronie alei prowadzącej do sanatorium „Borkowo”, na wysokości zalewu) położone jest charakterystyczne wzniesienie o łagodnych zboczach, zwane Trzy Garby (138,5 m nad poziomem morza).

Po przejęciu władztwa nad miastem od Wedlów przez ród Manteufflów doszło do złączenia dotychczasowego herbu z herbem rodowym nowych właścicieli – czerwona belka w srebrnym polu.
Najstarsza zachowana pieczęć miejska przedstawia pionowo podzieloną tarczę, w prawym polu widać poziomy pas z czterolistną różą, a w polu lewym gałąź podobną do wawrzynu, z liśćmi i okrągłymi owocami. Na obwodzie pieczęci, napis: SECR CIVITATIS PVLZIN. Inna zachowana pieczęć, z XVI wieku, przedstawia w polu prawym szeroki pas, w polu lewym, na trzech pagórkach, trzy winne latorośle. Na pieczęci znajduje się łaciński napis: „Sigillum Oppidi Polzin” (Pieczęć grodu Połczyn).
Najstarszym zachowanym herbem Połczyna Zdroju jest ten znajdujący się na ścianie budynku Urzędu Stanu Cywilnego przy Placu Wolności. Jest najbardziej okazały i najwyraźniej przedstawia barwy i treść symbolu miasta Jest też najbardziej podobny do opisywanego w herbarzu z 1898 roku pt. „Herby miast Prus Wschodnich”. Otto Hupp nieco inaczej, niż jego poprzednik opisał herb miasta – „w srebrnym polu dwudzielnym w słup, z przodu (po stronie prawej) czerwona belka, z tyłu (po stronie lewej) na trzech wzgórkach, trzy krzewy winne z niebieskimi gronami. Belka jest z herbu Manteufflów, którzy od końca XIV do XIX wieku posiadali prawo własności Połczyna”.
Posługując się tym opisem Rada Miejska w listopadzie 1994 roku uchwaliła, iż herbem miasta jest „tarcza herbowa dwudzielna w słup, po prawej stronie w polu srebrnym pas czerwony. Po lewej stronie w polu srebrnym, na trzech zielonych wzgórzach trzy krzewy winorośli z winnymi gronami niebieskimi, wsparte na tyczkach ze złota”. Odmianą uroczystą jest herb wielki, gdzie „na tarczy spoczywa korona złota, trzymana przez dwa wiekowe dęby”. Poniżej tarczy złota wstęga z łacińską dewizą „SALUS PUBLICA SUPREMA LEX” (Dobro publiczne najwyższym prawem).

Nach historischen Quellen, ursprünglich im Wappen der Stadt, gab es ursprünglich drei Weinreben mit hängenden Weintrauben. On the circumference of the seal, the inscription: SECR CIVITATIS PVLZIN. Another preserved seal, from the 16th century, depicts a wide belt in the right field, three vineyards in the left field, three vineyards. On the seal there is a Latin inscription: "Sigillum Oppidi Polzin" (Seal of the Połczyn castle).
