L'edificio, risalente al sec. XVIII, è situato nella località di Rivalta, borgo rurale, ora disabitato, posto a Nord-Est di Pocenia, tra il fiume Torsa, a Nord della loro confluenza.
Si tratta di un edificio, derivante da un originario impianto del sec. XV, in muratura mista in laterizi e ciottoli intonacata e parzialmente tinteggiata, con manto di copertura in coppi.
La facciata principale è molto semplice e priva di decorazioni, essa presenta una porta centrale rettangolare, due finestre rettangolari al lati ed una monofora campanaria sulla sommità.
Sul lato destro sono presenti due aperture semicircolari lungo la parete dell'aula. Un'apertura di uguale forma presente nel presbiterio è stata recentemente murata. L'aula è rettangolare con travi a vista nel soffitto, mentre il presbiterio è quadrato con volta a crociera.
La cache è di dimensioni micro, portatevi una penna e buona caccia.
